• Applicant’s Birth Certificate
• Recent (within the past three months) passport-size photo with white/red/blue background of the applicant
• Applicants passport showing a valid visa (if applicable)

1. The online application is available for PN, K1 – K3 Classes.
網上申請適用於報讀預備班、幼兒班、低班及高班。

2. The fields on the application form marked with a red asterisk * are mandatory fields. Leaving the mandatory fields empty will result in failure of submission.
申請表上註有紅色星號*的地方是必需填寫的欄位。請勿留空任何標明「此欄必須填寫」的欄位。

3. Once your application has been submitted, please save and print the file for record. You will be given an application number for future enquiries and correspondences with us.
提交申請後，請保存並打印文件以作記錄，閣下將獲得一個申請號碼作將來查詢和聯絡之用。

4. Please make sure your email address is correct for receiving the confirmations and updates from us.
本校將透過電郵聯絡家長及不定期發放學校資訊，請確保閣下的電郵地址正確。

5. Please make sure your personal email settings will not block correspondence from SDM-Chatsworth International Kindergarten.
請確保閣下的個人電子郵件設置不會截取新加坡卓薈國際幼稚園的郵件。

6. In compliance with the regulations of the Hong Kong Education Bureau, non-local children may reside in Hong Kong for the purpose of education only with the approval of the Director of Immigration. We do not accept applications from children without approval.
根據香港教育局規定，非本地出生之幼兒只能在獲得入境事務處處長批准的情況下才可在香港接受教育，本校不接受未經批准之非本地出生幼兒的入學申請。
